Palliative Care Nurse Practitioners: An extension of existing specialist palliative care services at St Vincent’s Private Hospital Brisbane
At St Vincent’s Private Hospital Brisbane, we have a dedicated team of health professionals, who are committed to providing comprehensive, specialised care to people with advanced, progressive life-limiting illnesses. We are excited to announce the introduction of a Palliative Care Nurse Practitioner, as an extension to our existing specialist palliative care services.
Our Palliative Care Nurse Practitioner offers unique and highly advanced nursing skills and will support patients with complex palliative care needs using advanced nursing knowledge, expertise and evidence-based practice.
Our Palliative Care Nurse Practitioner supports patients across a range of settings including home, hospital, residential aged care and medical clinics.
The Nurse Practitioner works autonomously and collaboratively within our Palliative Care Team as well as with other health care professionals (e.g. General Practitioners) and providers (e.g. hospitals and community services) to assist with the timely provision of appropriate care and support.
Our Palliative Care Nurse Practitioner supports provision of quality care through:
- Assessment, management and evaluation of patients
- Advanced nursing care for patients with complex needs
- Direct patient referral to other health care professionals
- Prescribing medications
- Ordering and interpreting diagnostic investigations
- Delivering personalised care in the home
- Providing continuity of care between hospital and home
- Providing advice and support to patients and their loved ones (including care for physical, psychological and spiritual needs).
This new initiative will enhance our existing specialist palliative care services available to patients and their families.
