Bringing Care Home brings to life our commitment to deliver better and fairer care for all, by reimagining how and where we deliver services across our network.

Aligned with our Better and Fairer 2030 Strategy, this initiative advances three core strategic priorities: enhancing impact by improving access and quality of care; connecting care by leveraging digital and home-based models; and transforming the health system to better serve all Australians, especially those currently underserved.  

This initiative reflects our goal to:  

  • Create a more connected, person-centred model of care – one that integrates hospital, community, and virtual services.  
  • Improve equity and access, particularly for those who live outside major centres or face barriers to traditional care.  
  • Use innovation and digital technology to enhance care quality, efficiency and reach.  
  • Strengthen our Mission by extending our care beyond hospital walls. Bringing compassion, dignity and expertise directly into people’s homes and communities.  

Through Bringing Care Home, we will continue our long tradition of going where the need is greatest, expanding Home and Virtual Care to make support more accessible, compassionate and efficient. By building on our strengths in digital innovation and our connected “One St Vincent’s” approach, we’re delivering care that is better, fairer and more responsive to people’s lives.

Our Key Focus Areas

Acute Care at Home

Delivering hospital-level care at home, including pre-admission support, post-operative recovery, and acute care in the home. 

Chronic Condition Management at home

Helping people live well with chronic conditions such as diabetes, heart disease, and respiratory illness, using wearable devices, 24/7 health monitoring, and a dedicated virtual care team. This integrated model will detect, treat, and manage health issues early, reducing hospital visits and improving long-term outcomes. 

Ageing and Residential Aged Care (RAC) Virtual Support

Supporting older Australians to receive high-quality care at home or in their RAC facility, minimising hospital transfers and enabling quicker returns from hospital stays.
